Since 1994, P-B Health has provided in-home medical and personal care services to the elderly, disabled, infirmed and special care patients, including those who require in-home post surgical care. With a unity of focus, the entire team at P-B Health responds to the health care and quality of life concerns of people in many communities in the Baltimore Metropolitan area. Following its directive to deliver high quality care with great compassion, the nurses, therapists and home health aides at P-B Health reach many of the most underserved communities in the Baltimore Area. P-B Health is Medicare and Medicaid certified and licensed. The agency is accreditedby the Joint Commission. The agency is also a member organization of the Maryland National Capital Homecare Association and the National Association of Home Care.
P-B Health provides services in Baltimore City, Anne Arundel, Baltimore and Howard Counties; and specializes in providing services to underserved communities.

SERVICES
Home Health Services
Our Home Health Services comply with rigorous guidelines provided by Federal and State regulations. All of our patients receive the same OASIS assessment as required for Medicare and Medicaid patients and all have a physician approved plan of care for sixty day episodes.
Skilled Nursing Care
Skilled Nursing Care is provided for patients who require intensive medical support following an illness or accident. P-B Health offers the following skilled nursing services:
- Wound Care and Management
- IV Therapy
- Catheter Care and Training
- Diabetic Care and Education
- Cardiac Care and Restorative Functioning
Therapy Services
We provide Occupational, Physical, and Speech Therapy services for our patients as well. All of our therapists are board certified and licensed, while keeping their skills current by attending in-service trainings. Some therapy services provided are:
- Gait and balance training
- Personalized rehabilitation exercises
- Assistive device training
- Home environment inspections
Medical Social Work Services
Patients sometimes need the help of a Social Worker to ensure that the Home Health care plan that the doctor approves will actually be sustainable. The Social Worker might assist the patient with the following:
- Assist with Creating Advance Directives
- Resource Coordination
- Utilizing Community Resources
- Short-term Counseling
- Assess for Financial Eligibility of Additional Health Resources
Personal Care Assistance
Assistance with personal care moves people closer to independent living. Home Health Aides offer help with daily household maintenance and personal hygiene activities. Personal Care Assistance may include:
- Respite Care
- Hygiene and Grooming
- Meal Preparation
- Light Housekeeping
- Assistance with Ambulation
Supplemental Services
The physical health of our clients involves more than just nursing care. In addition to our professional staff of highly qualified clinical nurses, therapists and compassionate Home Health Aides, P-B Health also coordinates patient care in the following supplemental areas:
